The best way to start the process is to complete the Appointment Request form. I review these as they come in and can make a determination about scheduling at that point. If I am unable to accommodate an appointment within a week or two, I will likely refer you to some alternate contacts. Appointments can open up so if you continue to have difficulty finding a provider after a few weeks, I encourage you to submit the request again.

Boston (Main Office) Information
Parking: There is metered parking on the street in Boston ($3.75/hour) It is best to arrive early to secure a spot..
Commuter Rail/T Station: The closest MBTA commuter rail stop is “Back Bay” and the closest T station is Copley or Arlington.
Entrance: When arriving at 82 Marlborough St, you will require an access code to enter the building. Upon your appointment confirmation you will receive the access code. The outer alcove door is unlocked and turns to the left. You will need to PUSH the front door hard, (it is heavy) but not locked. The inner door requires an access code.
Waiting Areas: Please make yourself comfortable in either the large lobby waiting area on the first floor entrance level or in the small waiting area outside the elevator on the second floor. I will greet you at the time of your appointment.
Restrooms: You will find a restroom on the Main Level to the left of the Lobby waiting area and there are two restrooms at the top of the stairway on the second floor.
Office: From the front waiting area, go left and take the elevator or steps to the second floor. There are no numbers on the office doors, just name plates.
