If you would like to refer a client , kindly review my  Office Policies and feel free to complete the brief  referral form.

I will then contact the client and be in touch with you shortly in order to coordinate care or to confirm the referral outcome.   Please be sure to check the Practice Update page in the event that I am away, unable to take new referrals or closed due to weather emergencies.
